We are hiring aRegistered Nurse (RN) for aHome Health position providing skilled nursing care to patients in their homes in the Plainfield area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Perform patient assessments, medication management, and care planning

• Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• One year of RN experience and the clinical expertise to deliver quality patient care
