Primary Care Nurse Practitioner or Physician Assistant

St. Mary Internal Medicine is an independent adult primary care practice in Plainfield, Illinois, looking for an experienced Nurse Practitioner or Physician Assistant to join our growing team.

We’re looking for someone who enjoys building long-term relationships with patients and wants the flexibility and personal environment of an independent practice rather than a large health system.

• $120,000–$132,000 annual salary for full-time
• Full-time or part-time opportunities
• Established primary care patient base
• Opportunity to develop lasting relationships with patients and have a meaningful role within the practice

• Provide outpatient adult primary care
• Evaluate and manage acute and chronic conditions
• Perform routine preventive and wellness visits
• Order and interpret appropriate labs and diagnostic testing
• Develop treatment plans and coordinate ongoing care

• Illinois-licensed FNP, AGNP, or PA-C
• DEA registration and Illinois controlled-substance license
• Comfortable practicing adult outpatient primary care
• Primary care or internal medicine experience preferred
• Spanish speaking is a plus
