- Conduct thorough assessments of patients, identifying and managing medical conditions under the supervision of a Physiatrist.
- Develop and implement individualized care plans, focusing on function and rehabilitation to improve patients' quality of life.
- Collaborate with the healthcare team to ensure comprehensive care, including medication management and therapy recommendations.
- Educate patients and their families on treatment plans, health maintenance, and preventive care.
- Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
- Licensed Nurse Practitioner with a valid New Jersey certification.
- Experience working independently while effectively managing your own caseload.
- Submit accurate and thorough clinical documentation promptly, ensuring compliance with company policies and regulatory requirements.
