Job SummaryDaVita Kidney Care is bringing our industry‑leading clinical quality to patients on‑site at skilled nursing facilities (SNF) through our new team, DaVita Skilled Nursing Dialysis Services. This role provides comprehensive dialysis care to patients residing in SNFs, coordinating with SNF staff and leveraging DaVita’s resources to support efficient and high‑quality dialysis treatment.
Responsibilities- Patient education: Educate patients and their families in the SNF setting and conduct impactful and meaningful education sessions.
- Direct patient care: Independently demonstrate best practice techniques and deliver optimal dialysis care, supervising Dialysis Technicians as needed.
- Complex case management: Meet or exceed clinical outcomes; multitask to meet the needs of patients, the program, and our SNF partners.
- Autonomy & critical thinking: Provide comprehensive patient care in an independent setting, assess, diagnose, and implement complete care plans, and evaluate patient status continuously.
- Coordinate care with SNF staff as needed, ensuring dialysis treatment stays on track.
- Required: Current Registered Nurse (RN) license in the state of practice; minimum 12 months of RN experience.
- Associate’s Degree in Nursing (ADN) from an accredited school of nursing required;
Bachelor of Science in Nursing (BSN) acceptable; three‑year diploma from an accredited program may substitute a nursing degree. - Basic computer skills and proficiency in MS Word and Outlook.
- Nice‑to‑haves: Previous chronic hemodialysis, acute dialysis, and/or peritoneal dialysis (PD) nursing experience.
- Training/teaching/healthcare education background.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).

Wage range: $44.00-$61.00 per hour.
