Overview
Hartford Healthcare seeks a compassionate Revisit Nurse to provide high-quality follow-up care for patients across our Connecticut network. In this role, you will perform ongoing assessments, deliver direct nursing care, and coordinate services after initial treatment or discharge. Responsibilities- Perform ongoing patient assessments and deliver direct nursing care after treatment or discharge.
- Monitor symptoms and manage medications as part of follow-up care.
- Provide wound care as needed and coordinate post-discharge services.
- Educate patients and families about care plans, medications, and self-management at home.
- Document all care and updates in the electronic health record (EHR).
- Collaborate with an interdisciplinary team to ensure comprehensive, continuous care and appropriate referrals.
