Job DescriptionMetro is looking for a motivated and passionate per diem Physical Therapist to provide in‑home care. - Preference given to see patients in your primary geographic area
- Accept and treat patients within your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family
- Plan, prepare and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, home health aides, and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
- Physical Therapist Program graduate
- Licensed Physical Therapist in the state of New York or eligible
- Home health experience preferred but not required – a mentor will assist with documentation and compliance
- Ability to travel to patients’ homes within the assigned geographic territory
