Job Definition

The role of the Outreach Coordinator is to assist with the promotion of Project Forward, the Agency, and its programs to the community. The Coordinator will work with existing agency personnel and a variety of outside agencies to provide education on the availability of healthy relationships workshops and other substance use/mental health prevention services through the agency.

Educational Requirements

High School Diploma or equivalent

Experience Required

One to four years of related human services experience. Knowledge of community resources/entities and mental health and recovery services.

Duties And Responsibilities
  • Act as liaison between the Agency and the communities it serves.
  • Represent the Agency in a professional and knowledgeable manner.
  • Assist in the promotion of the Agency through information and education about its services to the communities we serve.
  • Provide direct outreach to courts, attorneys, hospitals, probation, parole, DSS, CPS, DDP, PCPs, schools, and various other community groups with direct referral back to the Agency and its programs.
  • Represent the Agency at various community events such as conferences, trainings, fairs, forums, etc.
  • Assist in the creation of flyers and promotional material under the direction of the director(s).
  • Document and report on contacts made in the community, including name of contact, agency (if applicable), address, and telephone number.
  • Assist the Intake Specialist and Program Manager with contacts to individuals reached during events/engagements and document on the appropriate tracker.
  • Engage with community members and schedule attendance at events or presentations about program offerings (e.g., universities/colleges, libraries, medical agencies).
  • Schedule indoor and outdoor events for Project Forward staff and other CNG events.
  • Organize all items, flyers, and give‑aways for events.
  • Manage and lead staff who volunteer at events.
  • Monitor and manage give‑away supplies, notifying management when to reorder.
  • Facilitate the sharing of data with individuals to aid partnered decision‑making and keep clients informed of progress.
  • Complete QI training during onboarding and participate in QI refresher training as needed.
  • Cultivate a culture of inclusion for all employees that respects their individual strengths, views, and experiences, driving better decisions, innovation, and business results.
  • Adhere to all safety protocols and procedures to ensure a safe working environment.
  • Apply health and safety awareness protocols to maintain a safe work environment.
  • Other duties as assigned.
