You will work closely with our interdisciplinary team to develop and implement individualized care plans, monitor patient progress, and provide education and support to patients and families.

• Provide direct patient care, including assessing, planning, implementing, and evaluating patient care plans

• Develop and maintain strong relationships with patients, families, and healthcare team members

• Conduct initial and ongoing assessments of patients' physical, emotional, and social needs

• Develop and implements individualized care plans to meet the unique needs of older adults

• Collaborate with the interdisciplinary team to coordinate patient care and services

• Monitor patient progress, identify areas for improvement, and make adjustments to care plans as needed

• Educate patients and families on disease management, medication use, and self-care techniques
