Interim Health Care is seeking a dedicated LPN to join our team and provide one‑on‑one nursing care for an adult female client in her home in Plaistow, New Hampshire. Weekend Day Shifts

• Saturday & Sunday
• 8:00 AM – 8:00 PM
• 24 hours per weekend

• Focus on one patient at a time
• Build meaningful, long‑term relationships
• Deliver personalized, patient‑centered care
• Enjoy greater autonomy and clinical independence
• Make a direct impact on a patient’s comfort, safety, and well‑being

The client requires skilled nursing support and ongoing monitoring related to:

• Epilepsy and seizure management
• History of transient ischemic attacks (TIAs)
• Cerebral palsy
• Medication administration and symptom monitoring
• Ongoing health assessment and support

• Provide skilled nursing care in accordance with the established care plan
• Administer medications and treatments as ordered
• Monitor patient condition and promptly identify changes in health status
• Respond appropriately to medical concerns and emergencies
• Collaborate with physicians, nurse practitioners, case managers, and other members of the healthcare team
• Maintain accurate and timely clinical documentation

• Active Licensed Practical Nurse (LPN) license in the State of New Hampshire
• Home care experience preferred
• Current CPR certification required (renewal classes provided by Interim Health Care)
• Strong clinical assessment and communication skills
• Ability to work independently and confidently in a home setting
• Experience caring for patients with neurological conditions is a plus
