RN​/LPN

River Woods Exeter seeks a compassionate RN or LPN to provide high-quality care for residents in our continuing care retirement community on the 3p-11p shift. In this role, you will complete assessments, administer medications and treatments, respond to changes in condition, and collaborate with an interdisciplinary team to support resident-centered care. You will communicate with residents and families, maintain accurate clinical documentation, and support a safe, respectful environment.

Responsibilities

• Provide direct nursing care for residents on the 3p-11p shift
• Complete assessments and monitor resident conditions
• Administer medications and treatments per orders and protocols
• Document care accurately in clinical records and electronic systems
• Collaborate with interdisciplinary team members to coordinate care
• Communicate with residents and families about care plans and changes
• Respond to emergencies and follow safety and infection-control procedures
• Support a respectful, person-centered environment for seniors
• Supervise and support CNAs and other care staff as appropriate
• Participate in ongoing training and quality improvement initiatives
