Licensed Practical Nurse

River Woods Exeter, a leading continuing care retirement community, is seeking a compassionate RN/LPN for our overnight shift (11p-7a). In this full-time role, you'll provide high-quality nursing care to residents in assisted living and nursing care settings, including assessments, medication administration, and care coordination. Responsibilities

• Provide direct nursing care to residents during the 11:00 p.m. to 7:00 a.m. shift, ensuring safety, comfort, and dignity at all times.
• Complete ongoing assessments of residents' physical and emotional status, promptly reporting and documenting changes in condition.
• Administer medications and treatments in accordance with physician orders, facility policies, and state regulations.
• Monitor vital signs, manage chronic conditions, and provide appropriate interventions for acute changes in status.
• Perform skilled nursing procedures such as wound care, catheter care, and glucose monitoring; RNs may perform IV and more complex procedures per scope of practice.
• Accurately document all care, assessments, and interventions in the electronic health record (EHR) in a timely manner.
• Coordinate and oversee the work of CNAs and other support staff on the night shift, ensuring high-quality, resident-centered care.
• Respond to resident emergencies, perform first aid and CPR as needed, and follow established emergency protocols.
• Support residents with nighttime routines, including assistance with toileting, mobility, repositioning, and comfort measures to promote rest and prevent skin breakdown.
• Communicate effectively with residents and families, providing compassionate support and clear updates regarding care.
• Collaborate with the interdisciplinary team, including day and evening nursing staff, therapists, and physicians, to ensure continuity of care.
• Follow infection control, safety, and fall prevention protocols to maintain a safe environment for residents and staff.
