River Woods Exeter, a leading continuing care retirement community, is seeking a compassionate RN/LPN for our overnight shift (11p-7a). In this full-time role, you'll provide high-quality nursing care to residents in assisted living and nursing care settings, including assessments, medication administration, and care coordination. You'll collaborate with an interdisciplinary team, support residents' dignity and independence, and communicate with families. Responsibilities
- Provide direct nursing care to residents during the 11:00 p.m. to 7:00 a.m. shift, ensuring safety, comfort, and dignity at all times.
- Complete ongoing assessments of residents' physical and emotional status, promptly reporting and documenting changes in condition.
- Administer medications and treatments in accordance with physician orders, facility policies, and state regulations.
- Monitor vital signs, manage chronic conditions, and provide appropriate interventions for acute changes in status.
- Perform skilled nursing procedures such as wound care, catheter care, and glucose monitoring; RNs may perform IV and more complex procedures per scope of practice.
- Accurately document all care, assessments, and interventions in the electronic health record (EHR) in a timely manner.
- Coordinate and oversee the work of CNAs and other support staff on the night shift, ensuring high-quality, resident-centered care.
- Respond to resident emergencies, perform first aid and CPR as needed, and follow established emergency protocols.
- Support residents with nighttime routines, including assistance with toileting, mobility, repositioning, and comfort measures to promote rest and prevent skin breakdown.
- Communicate effectively with residents and families, providing compassionate support and clear updates regarding care.
- Collaborate with the interdisciplinary team, including day and evening nursing staff, therapists, and physicians, to ensure continuity of care.
- Follow infection control, safety, and fall prevention protocols to maintain a safe environment for residents and staff.
- Participate in shift-to-shift handoff, sharing relevant resident information and contributing to care planning.
