Summary

This role leads the on-site facilitation of system-wide, strategic clinical quality improvement projects. This individual builds relationships with multidisciplinary teams, providing consultation and support to achieve measurable outcome improvements. This individual utilizes quality improvement methodology and tools, and techniques in clinical environments to achieve organizational goals. This includes collaborating with clinical leaders to develop aim statements, key driver diagrams, establish key metrics, and interpreting and presenting quality data using sophisticated data analysis.

This individual will be assigned varying projects in alignment with the needs of the organization and will apply project planning and management to effectively meet CHST quality strategic goals. Leads and/or participates in clinical effectiveness projects aimed at applying current evidence and best practice to standardize care through the development of clinical guidelines and protocols. This role also works in partnership with the health equity team to reduce healthcare disparities.

Responsibilities
  • Leads multidisciplinary teams through the development of quality indicators and identifies opportunities for improvement, to monitor and evaluate the quality of care and services provided to patients based on evidence-based practices, regulatory and accrediting agency requirements, and data analysis of high-risk problem prone, and high-volume processes.
  • On-site Facilitation of improvement projects using scientifically proven quality improvement methodology with the ability to rapidly learn and apply (with guidance) improvement science principles, steps, and tools in the various activities.
  • Provides professional leadership, coaching and support for Hospital-wide and departmental quality improvement programs, projects and initiatives.
  • Able to communicate effectively, clearly concisely, and persuasively at all levels of organization, demonstrating a willingness to listen, respond appropriately and effectively navigate conflicts.
  • Develop and maintain professional, collaborative relationships with internal and external stakeholders (ie. Physicians, Clinical leaders, data analytics, patient safety, etc) to ensure alignment in achieving quality outcomes.
  • Develops and delivers presentations to groups in a training or facilitation role. Collaborates with QI/PI consultants and advisors and assists with developing presentations and documents that articulate a compelling story line through the effective use of data.
  • Collaborates with system quality director to develop quality improvement curricula, design program interventions and data strategy to increase expertise throughout the organization.
  • Analyzes quality and safety performance data against internal and external benchmarks to identify opportunities for improvement;
    Facilitates teams through development of internal and benchmarks when necessary.
  • Able to perform complex project tasks, prioritize multiple projects, and work under pressure to meet deadlines in a fast-paced environment.
  • Uses sophisticated data analysis methods within quality improvement projects (ex: run charts, process control charts, and other statistical analysis).
  • Facilitates the development of clinical guidelines and protocols – including leveraging order sets, Best Practice Alerts, and other electronic forms.
