• Assess patient needs and develop individualized treatment plans based on physician orders and interdisciplinary team input.
• Provide direct speech therapy services to patients in their homes or assisted living facilities within a defined geographic area.
• Ensure timely documentation and compliance with all Paradigm Home Care policies and procedures.
• Collaborate with physicians, nurses, and the care team to support optimal patient outcomes.

• Master's Degree in Speech-Language Pathology from an accredited university
• Active Speech-Language Pathologist license in the State of Texas
• Current CPR certification
• Valid Texas Driver's License and automobile insurance
• Minimum 3 years of experience as a Speech-Language Pathologist, with at least 1 year of recent home health experience (within the last three years)
• Knowledge of Well Sky EMR software desired

• Self-Motivated and able to work independently while meeting required deadlines
• Clear communicator, both written and oral
• Respectful, professional, and courteous in all client, team, and public interactions
• Collaborative team player with a patient-first mindset
