Home Health Aide

We are looking for a personal Home Health Aide & Caregiver for an elderly gentleman in his early 80s, where your dedication and skills will make a meaningful difference in the life of individual requiring personalized care. In this vital role, you will provide essential support to patient at his personal residence or if in the rehab setting, promoting independence, safety, and comfort.

Your proactive approach and caring nature will help enhance quality of life for him, ensuring he receives respectful, attentive, and professional care every day. This position offers a rewarding opportunity to work closely with someone with disabilities, facing chronic and memory-related conditions.

• Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient health by observing vital signs and reporting changes to the family / person in charge at home
• Support medication administration following prescribed guidelines while maintaining accurate medical documentation
• Help with meal feeding and providing guidance to him / his family with nutritional needs to promote overall well-being
• Maintain a clean and safe environment by assisting with light housekeeping duties like laundry and general cleaning
• Support patients at his home while adhering to HIPAA regulations to protect patient privacy
• Collaborate with his healthcare providers to ensure comprehensive patient care plans are followed accurately

• Prior experience working in assisted living facilities, nursing homes, or home health environments preferred
• Demonstrated ability to work effectively with individuals with developmental disabilities or complex medical needs
• Knowledge of medical terminology and medical documentation practices essential for accurate record keeping
• Certifications such as CPR (Cardiopulmonary Resuscitation), first aid, or specialized dementia/caregiver training highly desirable
• Long-term care experience that highlights your ability to provide consistent, compassionate support over time
