We are seeking a Part Time to grow to Full Time, on-site Case Manager to join our team in Plano /Lewisville, TX. The Case Manager will oversee the coordination of client care, ensuring personalized service plans meet individual needs. Responsibilities include conducting initial client assessments, maintaining ongoing communication with clients and families. The role also involves regularly evaluating client satisfaction, ensuring compliance with company policies and applicable regulations.

• Strong communication, interpersonal, and relationship management skills to interact effectively with clients, families, and caregivers.
• Exceptional organizational and manage service plans and caregiver assignments efficiently.
• Problem-solving and critical-thinking skills to address client needs and resolve challenges promptly.
• Knowledge of the home care industry or prior experience in caregiving services is highly advantageous.
• Proficiency with basic computer applications such as Microsoft Office and scheduling software.
• Ability to work independently and as part of a team in a fast-paced environment.
• Prior experience in Home Care a plus.
• High school diploma or equivalent is required; a bachelor's degree in healthcare management, or related fields is preferred.
