Position’s RoleSummary:
North Texas Behavioral Health Authority (NTBHA), based in Dallas is responsible for developing a collaborative system of care for children, adolescents and adults with severe mental illness and other serious emotional disturbances, as well as those experiencing a substance use disorder. This position’s actions and work must align with NTBHA values and goals, regulations, and policies and procedures, and meet all documentation standards and regulatory requirements.
The State Hospital Step Down Program is designed to identify, assess, and facilitate the successful transitions of adults with serious mental illness (SMI), or a combination of SMI and medical needs exceeding the supports available in traditional settings who are clinically appropriate for transition from a hospital-based setting to a community-based services with proper supports.
The State Hospital Step Down Program Manager will oversee all aspects of the program including training, supervision, deliverables, and all roles listed for the manger in the Supervised Living Group Home Standards. The State Hospital Step Down Program Manager provides direct supervision to members of the State Hospital Step Down Program Care Coordinators or are part of the Intensive Transition Team (ITT) and oversees admissions into the program, including the residents of the Transitional Home as well as those residing in the community.
The Program Manager is responsible for ensuring the services delivered after enrollment, including oversight of the daily activities within the home. The ITT will plan for and complete the person-centered step-down/transition process. The ITT shall collaborate with, as applicable or determined necessary based on clinical presentation and client preference: state hospital staff, NTBHA’s continuity of care staff, Medicaid Managed Care Organization staff, a peer support specialist, a substance use counselor, the participant, and the participant’s family or legally authorized representative.
The Program Manager represents the ITT in court-related processes including written reports and testifying, as approved by the Program Manager of State Hospital Step Down and/or Chief Clinical Officer.
Travel is required as part of this role’s duties. Travel can vary from 20% to 100% per day. This would include all counties within NTBHA Catchment area (Dallas, Ellis, Hunt, Kaufman, Navarro & Rockwall Counties). This role may be required to use a fleet vehicle and/or a personal vehicle for business needs. This may include, but is not limited to field & home visits, offsite meetings, co-located sites and/or pick up items for business needs, etc.
If your program requires transport of individuals, this would only be allowed with a company vehicle and approval from management.
This position is classified as exempt and requires flexibility in scheduling to meet organizational needs, including adjusting work hours as necessary to ensure job responsibilities are completed.
Qualifications Qualifications required:- Graduated from an accredited college or university with a master’s degree with a major specialization in Human/Behavioral/Social Services or related field.
