Position Summary
Primary function is to facilitate the patient care process, coordinate care with the interdisciplinary team, and the referral source.
Essential Duties And Responsibilities- Receive and appropriately document new referrals in a timely manner
- Participate in coordinating care with Nurse Supervisors and patient interdisciplinary team
- Provide effective communication to patients, staff members, other health care professionals and referral sources
- Respond to agency and patient needs in a professional and creative manner
- Schedule and appropriately document patient schedules in a timely manner
- Monitor full-time staff points and report to DON deviations from established guidelines
- Participate in coordinating care with management and patient interdisciplinary team
- Ensures accurate entry, access and retrieval of data base information
- Verifies insurance benefits
- Pre-certifies initial insurance coverage; follows up to maintain required re-certification coverage authorization; enters patient information regarding authorization/coverage approval
- Performs special projects and other duties as requested
- May assist with billing, payroll, and filing procedures
- Promotes agency philosophy and mission by presenting a positive image to patients, families, physicians and community agencies
- Demonstrate commitment, professional growth and competency

- Education:
Graduate of an accredited school of vocational nursing - Licensure:
Current Texas State Licensed Vocational Nurse - Experience:
One year experience as a Licensed Vocational Nurse, home health preferred
