Role Description

The Lead Clinical Intake Assessor/Licensed LCSW is a full-time, on-site role based in Plano, TX. This role is responsible for conducting comprehensive biopsychosocial assessments, determining appropriate levels of care, and developing initial treatment recommendations for new and returning clients. Day-to-day responsibilities include leading intake processes, coordinating with therapists and other providers, documenting clinical findings in accordance with ethical and regulatory standards, and ensuring timely follow-up for clients.

The Lead Clinical Intake Assessor will also help refine intake workflows, support quality improvement initiatives, provide clinical guidance to staff involved in triage and screening, and actively contribute to a supportive, trauma-informed environment. Collaboration with families, community partners, and multidisciplinary team members is integral to this role.

Qualifications
  • Active Licensed Clinical Social Worker (LCSW) credential in the state of Texas, with strong clinical assessment and diagnostic skills.
  • Experience conducting biopsychosocial assessments, risk evaluations (including suicide and safety assessments), and developing treatment plans in outpatient or community mental health settings.
  • Knowledge of trauma-informed, evidence-based, and culturally responsive therapeutic approaches, with the ability to work with diverse age groups and backgrounds.
  • Strong communication, documentation, and organizational skills, including proficiency with electronic health records and secure digital communication tools.
  • Demonstrated ability to lead processes, collaborate within a multidisciplinary team, mentor others involved in intake, and manage multiple cases in a fast-paced environment.
  • Commitment to ethical practice, confidentiality, and adherence to all relevant legal and professional standards in mental health care.
  • Master's degree in Social Work from an accredited program; prior experience in intake, triage, or case management is highly preferred.
  • Comfort working on-site in Plano, TX and engaging with clients, families, and community resources to support a holistic care model.
