This PRN position offers the flexibility to create your own schedule—perfect for LVNs seeking work‑life balance or supplemental income

- Provide skilled in‑home nursing visits based on the plan of care
- Perform vital signs, medication administration, wound care, and catheter care
- Observe and report changes in patient condition to the RN Case Manager
- Educate patients and caregivers on medications, safety, and disease management
- Complete accurate and timely documentation
- Communicate effectively with the care team
- Active Texas LVN license (required)
- Home health experience preferred
- Strong clinical and communication skills
- Ability to follow care plans and adapt to patient needs
- Reliable transportation & valid driver’s license
- Kinnser/Well Sky EMR experience (preferred)
- $45 per visit
- Flexible, self‑managed schedule
- Independence in the field
- Meaningful, patient‑focused work
- Consistent patient volume available
- On the road – Plano TX and surrounding areas
