Hospice RN Case Manager – Full‑time

The Hospice Registered Nurse Case Manager provides nursing care in accordance with the client’s plan of care for patients of all ages in their place of residence, coordinates care with the interdisciplinary team, patient/family, and referring agencies, and assumes responsibility for coordination of care.

Responsibilities
  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient’s physical, functional, psychosocial, and environmental needs and ADLs, including risk for grief, cultural and spiritual, verbal and non-verbal.
  • Implement the individual plan of care and recommend revisions as necessary.
  • Consult and educate the patient/family regarding disease process, self‑care techniques, end‑of‑life care, nutrition, and dietary needs; provide training to other staff as needed.
  • Initiate appropriate preventative and rehabilitative nursing procedures.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.
  • Coordinate all patient/family services and prioritize needs with IDT members.
  • Apply a case‑management approach and refer to other services as needed.
  • Inform physician and other personnel of changes in the patient’s needs and outcomes of interventions, while evaluating patient and family response to care.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Assess the caregiver’s ability to meet the patient’s immediate needs upon admission and throughout care.
  • Evaluate own need for support and use identified systems to meet the need.
  • Communicate information using current processes and technology available to the organization.
  • Apply criteria for admission and re‑certification to hospice care to establish appropriate levels of care and patient eligibility.
  • Perform on‑call responsibilities and provide on‑call service to patients/families as assigned.
  • Supervise and evaluate the hospice aide care plan in conjunction with the clinician establishing it.
Educational Requirements and Experience
  • Graduation from an accredited School of Nursing.
  • Current RN license in the state of Texas.
  • Current CPR certification.
  • One year of experience as a Registered Nurse in a clinical care setting required, hospice preferred.
  • Specialized skills preferred in palliation and end‑of‑life.
