Part Time Day Shift - Two, 12-hour shifts per week with one weekend shift every other weekend

The Licensed Vocational Nurse  will provide andassistin direct patient care under the supervision of a Registered Nurse,APPor physician. The LVN will provide medical management of patients andassistwith environmental safety and crisis management for the patient population in accordance withthe legal scope of practice and within established standards of care,policies and procedures. The LVN willdemonstrateperformance consistent with the Mission,Visionand Strategic Plan of the organization,demonstratequality and effectiveness in work habits and clinicalpracticeand treat coworkers, patients,families and all members of the health care team with dignity and respect.

The LVN will partner with the treatment team to provide aconsistentlytherapeutic milieu.

The LVN will crosstrainacross ERC and Pathlight and may berequiredto float or be assigned to other locations/facilities within the Region. The LVN maybe required to work on-call shifts based on business needs,in accordance with applicable federal, state, and local labor laws.

Essential duties and responsibilities:

  • Performs all nursing care within scope of practice to assigned patients under the supervision of an RN, APP, or provider
  • Manages patient assignment within assigned shift
  • Performs and documents vital signs, responses to treatment, education and nursing care andassistswith other basic medical care tasks assigned
  • Reports abnormal changes in patients or patients’ health and mental status to RN or provider in a timely manner
  • Receives, places and documents provider orders/communication intimelymanner using approved abbreviations
  • Effectively labels,collect sand processes patient specimens
  • Administers medications, injections, places andmaintainsnasogastric tube feeds, and provides wound care according to facility and state regulations
  • Mayassistlicensed provider with patient examinations and treatments by positioning, restraining patients, andassistingas directed
  • Performs all aspects of patient care in an environment thatoptimizespatient safety and reduces the likelihood of medical/health care errors
  • Ensures that care reflects goals of patient outcome activities
  • Contributes to individualized treatment plans in conjunction with the ERC multidisciplinary team
  • Participates in daily controlled substance count
  • Aidesin transition/transfer of patients to and from other units or facilities
  • Collaborates with staff toassistwith transportation arrangements for patient appointments outside of ERC
  • Responds to emergency situations until emergency response team (911) arrives onscene

Musthaves:

  • High school diploma or GED (Required)
  • Active LVN license in the state job is held (Required)
  • BLS certification (Required)
