Treatment Coordinator

Life Care Centers seeks a Treatment Coordinator to lead and coordinate nursing treatment plans for senior residents in a rehabilitation and long-term care setting. In this role, you will assess patient needs, develop and update care plans, oversee treatments and medication administration, and collaborate closely with physicians, therapists, and families. You will ensure regulatory compliance, accurate documentation, and quality standards while supporting a patient-first, compassionate culture.

Responsibilities

• Assess residents and coordinate individualized treatment and nursing care plans
• Administer and oversee treatments and medications per physician orders
• Collaborate with physicians, therapists, and interdisciplinary team members
• Monitor patient progress and update care plans and documentation
• Ensure compliance with regulatory and clinical quality standards
• Educate residents and families on care plans and treatment options
• Support discharge planning and continuity of care
• Maintain accurate, timely electronic health records
• Participate in quality improvement and clinical audits
• Provide leadership and guidance to nursing staff during treatments
