Registered Nurse, Home Health

Responsibilities

• Assess patients to determine their care needs
• Complete initial and ongoing patient assessments
• Protect patient and staff privacy in accordance with HIPAA
• Provide nursing care according to the plan of care and physician orders
• Monitor patient conditions and report changes to the care team
• Educate patients and families on their care and safety
• Take appropriate action when patient conditions change
• Help ensure quality, safe, and continuous patient care

• Current State license as a Registered Nurse
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting
• Home Health experience is preferred, but not required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

Demonstrates expertise in patient assessment, nursing care delivery, and adherence to HIPAA regulations while ensuring quality and safety in patient care. Capable of educating patients and families, monitoring conditions, and effectively communicating with the care team.
