Humana Inc. seeks a Care Coach in Florida to provide proactive, patient-centered care coordination for high-risk seniors within Conviva Senior Primary Care. You will serve as primary contact, coordinate care across systems, and support transitions of care and social needs.
Travel to patients' homes, clinics and community settings is required. The role combines clinic-based and field work with 2 days on-site and 2 days in-home per week, Monday–Friday 8am–5pm, with potential for overtime.
