Conviva Senior Primary Care is seeking a Care Coach to provide proactive, patient-centered care coordination and social support for high-risk seniors. You will conduct structured interviews, perform home visits, and act as a liaison among patients, primary care, specialists, and community providers.
Minimum 3+ years in ambulatory or senior-care settings with direct patient care is required. Bilingual English/Spanish and licensure options are preferred as part of a broader care coordination
