Home Visitor for Clinton and Franklin Counties

Your role at BHSN:
As a Home Visitor for Clinton and Franklin Counties, your community and child-focused approach to care will help North Country children and their families partner with the community to create happy, health, safe and school ready children.
Work Schedule:
Monday - Friday 8:30am-4:30pm

What your day might look like:
- This is a home visiting position partnering with expectant and new parents in order to develop and perform community outreach and assessments
- Connecting families to resources in order to support their baby's needs in the areas of cognitive, physical, social and emotional development, health, safety and assist with bonding and attachment.
- Conducting quality assurance reviews to participant's files to ensure all documentation adheres to HFA and HFNY standards
Your skills and qualifications:
- High school diploma and seven years' related experience required; bachelor's degree plus three years' related experience preferred; college coursework counts towards related experience
- Demonstrable knowledge of local/community resources
- Ability to work a flexible schedule to meet the needs of the families served
- Strong engagement, communication, time management and organizational skills required
- Valid NYS driver's license and reliable transportation required for travel
