Position Summary

The MDS Coordinator (RN) is responsible for coordinating, completing, and submitting accurate and timely interdisciplinary MDS assessments and care plans in accordance with CMS RAI Manual regulations and all applicable federal and state requirements. This role is essential in supporting clinical excellence, regulatory compliance, and proper reimbursement.

Responsibilities
  • Conduct Care Plan meetings with residents and families
  • Ensure supporting documentation is present in the medical record for accurate MDS coding
  • Support and maintain tracking systems for MDS scheduling
  • Manage daily PPS meetings and weekly Medicare meetings, including setting Assessment Reference Dates
  • Ensure compliance with federal and state regulations related to the RAI process
  • Maintain current MDS status for assigned residents
  • Perform accurate and frequent data entry into MDS software systems
  • Code MDS accurately using medical record review, observation, and staff/resident/family interviews
  • Participate in quality assurance and performance improvement activities
  • Complete electronic submission of required documentation to state databases and other entities
  • Coordinate with the billing office during monthly close to ensure all required assessments are completed
Qualifications
  • Associate’s or Bachelor’s degree in Nursing from an accredited college or university
  • Current RN license in good standing
  • Minimum one year of experience with Medicare/Medicaid reimbursement systems and case management
  • Knowledge of state and federal regulations related to the RAI process and reimbursement systems
  • MDS experience: 1 year required
  • Strong organizational, documentation, and assessment skills
