Choice in Aging seeks a Registered Nurse Care Manager - Casual (RNCM) to support frail, low-income elders under the MSSP Medi-Cal Waiver. The RNCM assesses clients as a whole, identifies functional and health barriers, and coordinates services to promote safe community living.
Works with the CiA team to fulfill the mission of enabling aging with dignity in the community. Minimum qualifications include CA RN license, bachelor’s in nursing, and strong English communication.
