On-Call Runner Licensed Vocational Nurse (LVN)Job Overview
Sunbright Home Health & Hospice is seeking a dependable and compassionate On-Call Runner Licensed Vocational Nurse (LVN) to provide skilled nursing support to patients in their homes throughout San Joaquin and Stanislaus Counties
.
This role is ideal for an LVN who is flexible, clinically strong, and comfortable working independently while supporting patients and families during after-hours and urgent care needs.
ScheduleThis position follows a rotating one-week-on / one-week-off on-call schedule:
On-call begins Monday at 5:00 PM
Ends the following Monday at 8:30 AM
Includes evenings, overnight, weekends, and holidays during the assigned week
Holiday CoverageIf the office is closed for a holiday, the On-Call Runner LVN will be responsible for providing full-day coverage to ensure patient needs are met.
Key ResponsibilitiesProvide skilled nursing care in accordance with the patient’s plan of care under the supervision of an RN
Respond to after-hours patient needs including phone triage and urgent in-person visits (“runner” visits)
Perform nursing assessments, treatments, wound care, and symptom management as appropriate
Communicate patient condition changes promptly to the supervising RN and clinical team
Educate patients and caregivers on medications, symptom control, and supportive care
Complete accurate and timely documentation in the electronic medical record
Maintain compliance with all state, federal, and agency policies
Additional Coverage & FlexibilityAbility to provide additional runner coverage as needed when other on-call staff are unavailable due to illness, vacation, or scheduling gaps
Must be flexible and willing to assist with urgent patient visits to ensure continuity of care across the service area
QualificationsRequired
Current and active California LVN License
Current CPR/BLS Certification
Minimum 1 year of nursing experience (home health or hospice preferred)
Reliable transportation, valid CA driver’s license, and auto insurance
Ability to travel throughout San Joaquin & Stanislaus Counties
Strong communication, assessment, and time-management skills
PreferredHospice or home health experience
Wound care and symptom management skills
Community-based patient care in home settings
Independent on-call responsibilities with clinical team support
