The Registered Nurse (RN) at Axis Community Health plays a vital role in delivering compassionate, patient-centered care in a Federally Qualified Health Center (FQHC) setting. The position will be working collaboratively with providers, medical assistants, and other care team members. The RN provides clinical support including patient triage, health education, care coordination, and chronic disease management. This position supports underserved and diverse populations and promotes health equity through culturally competent care.

Graduate of an accredited nursing program. Associate Degree in Nursing (ADN) is required, Bachelor of Science in Nursing (BSN) is preferred.

Minimum of one (1) year experience preferably in a hospital or clinic setting.

Current active California Registered Nurse license.

Experience and knowledge of primary care medical services.

Ability to meet requirements of CPR, First Aid and other mandated programs.

Demonstrate clinical assessment skills at the level necessary to meet the job requirements.

Ability to make independent decisions when circumstances warrant such action.

Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to medical care.

Effective interpersonal skills and the ability to work with an interdisciplinary care team.

Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, and policies and procedures that are necessary for providing quality care.

Capable of researching new methods and principles to incorporate them into existing nursing practices.

Proven decision making and analytical skills.

Ability to efficiently carry out duties and assignments.

Ability to use the Axis practice management systems and Electronic Health Records (EHR).

Strong employee relations and interpersonal skills.

Excellent writing, business communication, editing, and proofreading skills.

Ability to interact effectively and in a supportive manner with persons of all backgrounds.

Proactive, self-motivated and able to work independently in a fast-paced environment as well as on a team with the ability to exercise sound independent judgment.
