The Wellness Nurse provides nursing care and supervision to residents in the community. This includes direct resident care, collaboration with other disciplines, referrals to community resources, and utilizing resource materials as needed. The role participates in care conferences and staff meetings, completes paperwork in a timely manner, and communicates with health care providers, Eskaton support staff, and other community personnel. The Wellness Nurse also coordinates and tracks training for community care staff.
EssentialJob Functions
- Comprehensive Nursing Care: Promote the success of others; demonstrate basic clinical assessment skills for initial and follow-up appraisal of resident status; possess knowledge of clinical disease entities; perform nursing treatments and procedures as prescribed; assist with administration and management of diets and medications; support rehabilitative and preventative aspects of care and ensure resident safety, comfort, and protection; supervise and direct care provided by staff;
participate in training of care staff; assist physicians or extenders in rounds and assessments (where applicable); assist with marketing to the community and tours as needed.
- Documentation: Document resident status changes, instructions, responses, and ongoing care plans; maintain written care plans timely and accurately; complete pre-placement and routine evaluations per Eskaton policy; follow documentation procedures per RCFE regulations using accurate medical terminology; maintain records related to staff education and training.
- Personal and Professional Development: Attend workshops and classes to develop knowledge/skills; conduct shadowing for new Resident Medication Assistants; participate in orientation programs and internal committees; participate in staff meetings regularly.
- Care Management: Communicate resident information at team meetings; coordinate resident care; establish short-term and long-term goals for total resident care and management.

Starting pay ranges from $27.90 - $39.03 per hour. Pay factors include scope, responsibilities, experience, education, skills, internal equity, and market considerations.
Qualifications Education- Valid California Licensed Vocational Nurse or Registered Nurse license.
