The primary purpose of the position is to provide assigned residents with quality routine daily nursing care in accordance with current regulations, established nursing procedures and as directed by your supervisors while delivering on the company core values of wellness, compassion, experience, and results.
ESSENTIAL DUTIES AND RESPONSIBILITIES Adherence to Policies –Must perform duties in a timely fashion, and within the prescribed sequences and schedules.
Must perform assigned tasks with minimum supervision with an awareness for and a willingness to perform other tasks, as needed.
Must respect all resident rights, including the confidentiality of resident care information.
Follow all established safety precautions when operating equipment and must report hazardous and defective equipment or conditions to your supervisor.
Must be cooperative with other departments and be courteous and respectful in dealing with them at all times.
Participate in and respond professionally to surveys (inspections) conducted by government agencies.
Report on time to work and work the full scheduled shift. Check with the charge nurse for any schedule concerns or variances.
Must always maintain a professional, positive, efficient and friendly demeanor when dealing with residents.
Assume the authority, responsibility, and accountability necessary to carry out the assigned duties.
Assist residents as needed with activities of daily living. This includes bathing, hair care, nail care, oral care, shaving, dressing, eating, pericare, assisting to and from bed, toileting, turning, transferring, lifting, changing when needed, etc.
Bathe each resident according to schedule. Change bed linens completely on bath days, more often as needed. Clean showers and tub rooms thoroughly before and after each use.
Make rounds regularly on each shift and assist each resident as needed, making sure they are safe, clean, and comfortable.
Always respect resident privacy and confidentiality including all details of resident care. Refer all questions regarding patient's status to the Charge Nurse and all questions regarding operation of care center to the Administrator.
Safeguard and use appropriately all resident belongings and appliances: hearing aids, eye glasses, dentures, clothing, personal property, etc.
Escort residents to scheduled activities (recreational, religious, etc.) according to care plan.
Bring residents to the dining area, serve trays, feed residents as needed, and provide assistance to those not totally independent. Pick up trays and replace in kitchen cart for depository. Document meal intake percentage.
Fill resident water pitchers with ice water at least once a shift. Offer fresh water routinely to each resident, and consistently upon entrance of the patient room.
Perform restorative and rehabilitative nursing tasks as assigned. This includes turning and massaging bedfast residents every two hours, giving exercises as instructed, ambulating, toileting every two hours or as ordered or needed.
Inform the Charge Nurse of any observed change in resident's condition. This includes decrease in level of response, alteration in bowel function, skin issues, changes in skin color or temperature, bruises, rashes, falls or other accidents, altered behavior, etc.
Accurately document all duties performed on the appropriate records. This includes meal intake and snack charting, TPR & BP, weights, ADL’s, bowel movements, accident reports, etc.
Use proper body mechanics and proper use & maintenance of lifts.
Complete ADL’s according to resident care plan goals.
Give accurate report to nurse and to on-coming C.N.A.’s
Observe safety rules, policies and procedures.
Wear the proper uniform including scrubs according to policy, name tag and gait belt at all times.
Chart appropriately and completely.
Instill positive relationships among the staff, with residents and with the families of residents.
Assume ownership of problems and facilitate solutions.
