POSITION SUMMARY/RESPONSIBILITIES
The CHF Clinical Coordinator is responsible for the coordination services provided to heart failure patients in cooperation with a multidisciplinary care team in the outpatient setting. Provide direction to patient care staff to promote quality patient care and nursing services on a continuous basis. The CHF Clinical Coordinator responsibilities include direct patient care, phone triage of patient questions utilizing physician protocols, orientation and training of clinical support staff and functions as a resource on clinical issues with regard to the CHF patient.
Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Provides patient education as directed by the physician.
EDUCATION/EXPERIENCE
Graduation from an accredited school of nursing with current RN licensure in the State of Texas, BSN preferred. Experience applying customer service behaviors and communication skills required. Minimum 2 years of recent clinical experience directly related to cardiology. Must meet career ladder Staff Nurse II criteria for worked area. Verification of course completion in accordance with all American Heart Association Basic Cardiac Life Support and Health Care Provider guidelines is required.
