Job Description
As an Ambulatory Licensed Practical Nurse, you will be an integral part of our interdisciplinary team, working collaboratively to deliver exceptional patient care. Your role involves utilizing the care process within your scope of practice, reinforcing patient and family teaching programs, and providing supportive and restorative care under the direction of registered nurses or licensed physicians. You will actively participate in performance improvement initiatives and strive for professional growth.
Responsibilities- Perform tasks and responsibilities within the framework of case finding and patient care.
- Reinforce patient and family teaching programs through health counseling and education.
- Provide supportive and restorative care as directed by registered nurses or physicians.
- Oversee the coordination of the interdisciplinary plan of care, ensuring consistency with the medical treatment plan.
- Document objective and subjective physical findings and interventions accurately.
- Administer patient care activities according to established standards, organizational policies, and procedural guidelines.
- Support performance improvement initiatives by participating in educational activities.
- Foster a culture of continuous professional growth and development.
- Maintain a safe and comfortable work environment, adhering to infection control protocols.
- Collaborate effectively with the interdisciplinary team to deliver holistic patient care.
- Graduate of an accredited school of nursing.
- Current licensure as an LPN in the State of New Jersey.
- Current BLS/HCP certification.
- Experience in a similar role or relevant specialty is preferred.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of an interdisciplinary team.
- Strong organizational and time management skills.
- Commitment to patient-centered care and continuous improvement.
- Willingness to adapt to changing work environments and patient needs.
- Ability to work evenings and weekends as required.
