- Accrued Vacation
- Assesses spiritual needs/resources of patients, families and caregivers and develops an individualized Plan of Care within 5 days of admission.
- Participates in the planning and implementation of the plan of care through the IDT process every 15 days.
- Manages a case load of 50 patients on average.
- Averages five visits per day.
- Assists patients and families in dealing with a terminal diagnosis and in understanding the dying process in the context of their belief system.
- Performs interdenominational services, funerals and committal services.
- Participates in On‑call coverage.
- Provides individualized religious and spiritual counseling services to clients regardless of religious or spiritual belief orientation.
- Accurately records and reports pertinent information regarding patients and families immediately after the visit or by the end of the workday.
- Helps to provide continuity of care between team members by ensuring verbal and written reports are clear, concise, and timely.
- Documentation is individualized and includes all required elements.
- Demonstrates a willingness to participate in community education by giving presentations and in‑services on spiritual issues.
- Provides supervision to seminarians and spiritual care students as requested.
- Serves as liaison with area clergy and faith communities.
- Helps agency staff and volunteers understand spiritual issues.
- Committed to Agency resolution against fraud and abuse.
- Knowledgeable in hospice philosophy and state and federal rules and regulations.
- Arrives to work and meetings on time and ready to work.
- Respects confidentiality of patients/families and other Agency employees.
- Attends educational meetings/conferences for professional growth as required.
- Sees change as an opportunity and maintains a level of flexibility, which allows for adaptation to new ways of performing.
- Follows directions and policies that allow for an organizational commitment to working under time pressures without sacrificing quality.
