The RoleFull-time nurse practitioner based in North Central Indiana, visiting approximately 20 patients daily across 1-2 facilities. Responsibilities include conducting exams, reviewing labs, managing medications and nutrition, consulting with providers, training staff, and documenting care. The position is consultative, focusing on recommendations rather than procedures, with a flexible, semi-autonomous schedule.
RequirementsIndiana-licensed NP/PA/APRN, DEA license, own vehicle, 3+ years in nephrology, geriatrics, or SNF settings preferred.
