A Care Coordinator is the family’s main point of contact who keeps everything organized and moving.

  • Coordinates care across caregivers, clinicians, facilities, and family members
  • Communicates consistently by text/email/calls to give updates, follow up, and prevent gaps
  • Supports families with decisions and helps them manage caregiver schedules, expectations, and performance issues
  • Assists with applications (Mass Health/PACE/LTSS and similar), including paperwork, document collection, and tracking progress
  • Completes/arranges in-home assessments (safety, function, psychosocial) and helps build/update the care plan
  • Monitors staffing and care delivery, escalating concerns to nursing/management when needed
