Location:

Community Based Recovery Services (CBRS), Plymouth

Education & Experience:

Bachelor's degree in a behavioral related field and 2 years of relevant work experience
Pay Rate: $28.20/hr-$31/hr (dependent on experience and education)
Statues/Shift: Full Time / Monday - Friday 8am - 4:30pm
Travel required with mileage reimbursement

About Us

High Point provides a range of community-based services including Community Based Recovery Support(CBRS). CBRSwas created to provide additional support to people as they work on their recovery within their communities. Often, clients who have been in an addiction treatment or Section 35 facility and are transitioning back into their communities'benefit from extra support.

High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness.

Community Support Navigator Requirements
  • Ability to work collaboratively with substance use and mental health disorder population, clients from diverse legal, social, economic, ethnic, cultural and racial backgrounds as well as family members, insurance providers and outside providers;
  • Possess the ability to maintain healthy boundaries;
  • Knowledge of appropriate confidentiality policies and procedures;
  • Good verbal and written communication skills;
  • Knowledge of basic office skills: phone, fax machine, copy machine, filing;
  • Knowledge of basic computer skills and software including Microsoft Word/Excel;
  • Ability to work independently without direct supervision while adhering to the program’s philosophical team approach;
  • Certification in HPTC’s approved de-escalation program as well as CPR;
  • Participation in annual TB screening.
Community Support Navigator Duties and Responsibilities
  • Compliance with Federal Regulations, HIPAA, 42 CFR Part 2, Confidentiality of Alcohol and Drug Abuse Patients;
  • Compliance to and working knowledge of all applicable HPTC policies and program procedures including but not limited to those relating to ethics/boundaries and duties of role;
  • Maintain safety and security of client/staff at all times;
  • Ensure timely contact of Clinical Director in event of a crisis/emergency;
  • Attend all mandatory trainings, staff meetings, and supervision meetings as required;
  • Provide client-centered Community Support Program (CSP) and Community Support Program Justice Involvement (CSP-JI) services as indicated in needs assessment/service plan (12 step meetings, linkages to medical/counseling appointments, court, housing etc.),
  • Provide coordination with HPTC outpatient units in region for client education/approach/sign-ups for CSP/CSP-JI services following Initiation policy;
  • Assisting members in improving their daily living skills;
  • Provide temporary transportation to essential medical and behavioral health appointments while transitioning to community-based transportation resources (e.g., public transportation, PT1 forms, etc.);
  • Encouraging and facilitating the utilization of natural supports and recovery-oriented peer-support and self-help support and services.
  • Provide 24 hour on-call coverage in program rotation;
