Registered Nurse – Home Health

Home Health Nurse

Provide comprehensive home health nursing care to patients following physicians’ orders, utilizing the nursing process to ensure quality outcomes. Coordinate patient visits and supervise home health aides and licensed practical nurses to maintain optimal patient care.

Responsibilities:

• Assess patient needs and develop individualized care plans based on physician directives and agency protocols.
• Administer treatments, medications, and wound care during home visits.
• Supervise and evaluate the performance of Home Health Aides and LPNs/LVNs.
• Document patient interactions accurately within the Home Care Home Base EMR system within 24 hours.
• Coordinate travel logistics for up to 75 miles within assigned territory, ensuring timely patient visits.
• Participate in weekly office meetings and adhere to agency policies and procedures.

Qualifications:

• Valid RN license in the state of employment.
• Minimum of 1 year of recent adult home health or hospice experience with demonstrated proficiency.
• Proficient in Home Care Home Base (HCHB);
  Oasis proficiency required for RNs.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel up to 75 miles from the care center and work flexible scheduling, including weekends and on-call rotations.
• Strong communication, organizational, and documentation skills.
