Activate Care in Massachusetts seeks an RN to travel to members' homes to complete in-person health assessments for a managed care plan's dually eligible members. This role focuses on assessment and documentation, not ongoing care management.
You will perform UCA/FA/Comprehensive assessments, document in Guiding Care, submit SOAP notes within 48 hours, and maintain high quality records. A valid MA RN license, current car insurance, and ability to work independently are required.
