Job Title: Hospice Certified Nursing Assistant (CNA)

Location(s): Plymouth, MI and Westland, MI (Plymouth & surrounding areas)

Employment Type: Full‑time Employee – 40 hours per week

Residential Home Health and Hospice is seeking a compassionate Hospice Certified Nursing Assistant to join our care team. The CNA will collaborate with colleagues to manage and deliver patient care plans in a supportive, team‑based environment.

• Provide direct patient care and support as outlined in individual care plans.
• Collaborate with interdisciplinary team members to ensure quality care delivery.
• Support patients in Plymouth and surrounding areas, including travel to patient locations.

• Certified Nursing Assistant (CNA) certification in the state of employment.
• High school diploma or equivalent preferred.
• Hospice experience preferred.
• Current driver’s license and ability to travel approximately 20% of the day to patient locations.
• Demonstrated ability to self‑manage time and schedule.
• Experience with tablets, mobile phones, and electronic medical record (EMR) software.
