Job Title: Hospice Admissions Registered Nurse (RN) – Field-Based | Per Admission Pay

Location: Greater Twin Cities Metro Area, 21-county service area (territories assigned based on coverage needs)
Schedule: Full-Time or Part-Time with 1 on-call shift per week (backup) and every 4th–7th weekend

- Ensure a smooth, compassionate transition into hospice care
. - Complete accurate, timely EMR documentation

- Experience in hospice, oncology, palliative care, or hospital-based end-of-life care
. - Strong ability to lead conversations and build rapport quickly
. - Confident clinical assessment and decision-making skills.
- Comfortable working independently in a mobile, field-based role
. - Reliable transportation and ability to travel throughout the metro.
