Position SummaryThe Licensed Practical Nurse (LPN) delivers skilled nursing care to clients in their homes and community settings, working within the Minnesota LPN scope of practice under the direction and supervision of a Registered Nurse. The LPN performs clinical assessments, administers medications and treatments, educates clients and families, and coordinates care with the broader care team. This role is ideal for a skilled, self-directed nurse who takes pride in building meaningful relationships with clients and supporting their independence at home.
Essential Duties and Responsibilities Skilled Nursing Care- Perform skilled nursing visits in accordance with the physician-ordered plan of care and within the Minnesota LPN scope of practice.
- Conduct clinical observations and assessments, including vital signs, wound assessment, pain evaluation, and functional status monitoring.
- Administer medications, including injections, IV therapy (where permitted under RN supervision), and other prescribed treatments.
- Perform wound care, catheter care, ostomy care, blood glucose monitoring, and other skilled nursing procedures as ordered.
- Provide compassionate, person-centered care that respects client dignity, preferences, and goals.
- Educate clients and family caregivers on disease management, medication regimens, treatment procedures, and safety in the home.
- Reinforce RN-developed care instructions and teach clients to recognize signs and symptoms that require prompt reporting.
- Support clients in building confidence and skills for self-management of chronic conditions.
- Report changes in client condition, newly identified needs, or safety concerns to the RN Supervisor promptly.
- Communicate effectively with physicians, pharmacies, home health aides, family members, and community partners.
- Participate in care conferences, team meetings, and handoff communications as scheduled.
- Coordinate with the scheduling team to manage visit schedules and respond to urgent client needs.
- Document all visits, assessments, interventions, and client responses accurately and on time in the electronic health record (EHR) per agency and regulatory requirements.
- Maintain compliance with Minnesota home care statutes, agency policies, and applicable state and federal regulations.
- Support agency readiness for Minnesota Department of Health (MDH) home care surveys.
- Follow agency infection-control, HIPAA, and safety protocols at all times.
- Current, unencumbered Licensed Practical Nurse (LPN) license in the State of Minnesota.
- Current Basic Life Support (BLS) / CPR certification.
- Valid driver’s license and reliable personal vehicle with current auto insurance.
- Ability to pass a Minnesota Department of Human Services (DHS) background study.
- Strong clinical judgment, time management, and independent decision-making skills.
- Effective written and verbal communication skills.
- 1+ years of LPN experience in home care, community health, assisted living, or skilled nursing.
- Experience with chronic disease management (diabetes, CHF, COPD, wound care) in a home or community setting.
- Familiarity with Minnesota home care regulations and the Home Care Bill of Rights.
- Experience using electronic health records (EHR) or point-of-care documentation systems.
- Agency orientation, including the Minnesota Home Care Bill of Rights, client rights, and agency policies prior to providing client care.
- Vulnerable Adults Act and maltreatment mandated reporter training.
- Infection control, standard precautions, and HIPAA training.
- Annual competency updates as required by agency policy and Minnesota home care regulations.
