We have a wonderful opportunity for an LPN who would like to have a meaningful role where they are valued every day. The position requires work every other Saturday/Sunday and may have on‑call shifts as needed.

• Provide compassionate and effective care and treatments to residents.
• Work with a team of professionals to ensure residents’ comprehensive needs are met.
• Review routines, calendar of events and resident needs.
• Ensure medications and supplies are ordered, stocked and counted.
• Ensure all charting and record keeping is accurate and complete.
• Attend meetings and trainings as required.
• Promote the Mission, Vision and Values of Transforming Age and our Culture of Excellence in all areas of responsibility.
• Perform other duties as assigned.
• Work every other Saturday/Sunday and may have on‑call shifts as needed.

• Graduate of an accredited LPN program.
• Hold an active LPN license in the state in good standing.
• Experience in a nursing home (assisted living or long‑term care preferred, but not required).
• Flexibility to work occasional weekend/holiday shifts.
• Knowledge of DHS regulations.
• Excellent interpersonal and communication skills.
• Ability to work well and collaboratively with team members and external individuals.
• Completion of a background check and drug screen.
• Other pre‑employment screenings may include physical, TB test, and fingerprinting per state/local guidelines.
