** Advanced Practice Provider – Speare Primary Care
** As a Speare Primary Care Advanced Practice Provider, you’ll manage your own panel of patients while collaborating closely with physicians and a dedicated care team. This role blends independence with genuine teamwork – you’ll build lasting patient relationships, handle the full scope of primary care, and contribute to a practice culture that puts patients and families at the center of every decision.

* What You’ll Do:

*** Evaluate patient history, perform physical exams, and order and interpret labs and imaging to guide accurate diagnoses and treatments
* Perform minor procedures for injury repair, disease prevention, and functional restoration, as granted through clinical privileges
* Develop individualized treatment plans and deliver care in both outpatient and hospital settings, within the Family Medicine scope of practice
* Consult with physicians and surgeons; recognize and respond to urgent conditions – initiating emergency care independently when needed
* Counsel and educate patients on their physical and mental health, ensuring they leave informed and empowered at every visit
* Actively contribute to quality improvement programs, helping the practice continuously raise the bar for patient experience
* Maintain thorough, accurate medical records; complete daily documentation and charges; and uphold strict confidentiality standards

* What We’re Looking For:

** Nurse Practitioner
* Master of Science in Nursing or Master of Nursing degree from an accredited institution
* NH Board of Nursing licensure to practice as a Nurse Practitioner Physician Assistant:
* Bachelor of Science degree and graduation from an AMA-approved Physician Assistant program
* NH Board of Medicine licensure to practice as a Physician Assistant Both:
* Minimum 3 years of experience as a Nurse Practitioner or Physician Assistant (3-5 years in a Primary Care setting preferred)
* Current BLS certification
* Excellent patient-facing communication and customer service skills
* Proficiency with electronic health records and standard clinical software
* Strong organizational, interpersonal, and independent decision-making skills
* Commitment to confidentiality and excellent patient experience.
