- Provide skilled nursing care to patients in their homes, primarily in Plymouth and surrounding areas
- Utilize your critical thinking and assessment skills to develop and implement individualized care plans
- Perform a range of essential nursing skills, including wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage
- Provide LNA supervision
- Work independently while being a valued member of our collaborative team
- Maintain accurate and timely documentation
- Participate in a rotating weekend and on-call schedule to ensure continuous patient support
- Travel to patient homes in various weather conditions
- Current New Hampshire Registered Nurse (RN) license
- Valid Driver's License and insured vehicle
- Minimum of one year of professional nursing experience
- Ability to work independently, strong self-motivation, excellent organizational skills, and the ability to travel
- Demonstrated proficiency in wound care, Foley catheter insertion and removal, PICC line and IV dressing changes, wound vacuum dressing changes, ostomy/ileostomy management, venipunctures, and Pleurex catheter drainage
- Associate's degree or higher in nursing
Demonstrates expertise in skilled nursing care, including wound care and catheter management, while effectively developing individualized care plans. Strong organizational skills and the ability to work independently are essential for providing high-quality patient support in home settings.
