* What You’ll Do:
*** Provide palliative care through evaluation and assessment of health history, diagnosing medical conditions.
* Develop individualized plans of care.
* Complete face to face assessments and prescribe medications or treatments.
* Collaborate with the interdisciplinary team.

*** Active NP certification (state-licensed).
* Valid driver’s license, reliable transportation, and current auto insurance.
* Strong communication skills and a team-oriented mindset
* Ability to work independently in home settings
* Commitment to patient-focused care

Salary: $108,000 - $125,000 annually Benefits:
