Prototypes is one of the nation's leading behavioral healthcare providers and is a "prototype" for such organizations. Prototypes Women's Center in Pomona is a 180-bed residential facility tailored specifically to women, including those who wish to involve their children in their journey to recovery. Our program aims to assist women in maintaining their progress in overcoming co-occurring disorders through a wide range of supportive services.

Children can foster a secure attachment with their mothers in a secure setting, while also accessing mental health support and educational assistance. Our interdisciplinary team of professionals provides comprehensive treatment for substance use disorders and mental health, parenting, educational, and life skills courses. Treatment can range from 60 days to 6 months.
The Intake Coordinator II is the first point of care for engaging clients in treatment at Prototypes treatment programs. The Intake Coordinator II is responsible for providing in-person initial assessment of candidates to determine the need for SUD services, completion of program admission forms, and initial Treatment Plan. The Intake Coordinator II provides a warm handoff to the client's treatment team, assuring new clients are stabilized and urgent needs have been addressed.

The Intake Coordinator II will work closely with Counselors, Case Managers, Therapists, and other team members to coordinate and monitor client issues. The underlying responsibilities for the Intake Coordinator II to the client population include assessment of needs, care planning, education, information, and referral.

KEY RESPONSIBILITIES Direct Service
• Provide comprehensive initial assessment of candidates' needs.
• Ensure clients' urgent needs are met and appropriate referrals have been made.
• Complete all documentation involved in the admissions process.
• Begin the process of assisting clients in applying for Medi-Cal and/or County funding, as applicable.
• Complete the Initial Treatment Plan, as applicable.
• Provide a warm handoff to assigned residential treatment staff upon completion of the admissions process.
• Provide follow-up to ensure services are obtained.
• Provide crisis intervention as needed within scope of practice.
• Develop collaborative relationships with community partners.
• Maintain frequent communication with the treatment team and engage in regular consultations, clinical team meetings, case conferences, and plan development meetings.

Clinical Documentation
• Write and complete all progress notes within 72 hours of service delivery.
• Maintain compliance with the due dates for all clinical documentation.
• Write client's progress letters and court reports and obtain final approval from the supervisor.
• Documentation must be maintained in compliance with agency policy and procedures, HIPAA, 42 CFR pt. 2, and SAPC standards.
• Completes all assigned URCs within a timely matter. Administration and Other Duties
• Maintain full compliance with BBS regulations.
• Attend and actively participate in Clinical Group Supervision.
• Actively participate in agency and team meetings.
• Participate in training opportunities and pursue certifications in evidence-based practices.
• Communicate collaboratively with all members of the behavioral health team including medical, mental health, psychiatry, substance use disorder, and other staff.
