Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system.

By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.

Prototypes is one of the nation’s leading behavioral healthcare providers and is a “prototype” for such organizations. Prototypes Women's Center in Pomona is a 180-bed residential facility tailored specifically to women, including those who wish to involve their children in their journey to recovery. Our program aims to assist women in maintaining their progress in overcoming co-occurring disorders through a wide range of supportive services.

Children can foster a secure attachment with their mothers in a secure setting, while also accessing mental health support and educational assistance. Our interdisciplinary team of professionals provides comprehensive treatment for substance use disorders and mental health, parenting, educational, and life skills courses. Treatment can range from 60 days to 6 months.

The Case Manager I provides in-person support to clients by coordinating care both within Health

RIGHT 360 and with outside service providers. They work closely with clients to complete intake paperwork, conduct outcome assessments, and apply for benefits. As part of a multidisciplinary team, the Case Manager I helps ensure effective treatment that aligns with Health

RIGHT 360’s mission, values, and policies. Services are delivered flexibly in outpatient offices, residential facilities, or in the community, depending on client needs and program requirements. Therefore, reliable transportation is required.

KEY RESPONSIBILITIES Direct Service
  • Assist the client with intake by completing case management assessment and entering financial and benefit information.
  • Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
  • Assess the client’s case management needs and complete all releases of information.
  • Connect the client to benefits, mental health, physical health, employment, probation, DCFS, housing, community resources, outpatient substance use disorder services, and aftercare.
  • Coordinate communication and external service linkage including assisting with scheduling appointments, communicating with probation, scheduling child visits, communicating with DCFS, obtaining all court minute orders, providing appointment reminders for therapy and psychiatrist.
  • Deliver services in outpatient, residential, or field depending on assignments and client need.
  • Complete the VI-SPDAT and connect the client to safe housing options.
  • Prepare the client for job search by assisting with resume writing and job search preparation.
  • Monitor client's progress toward achieving case management treatment plan goals and provide treatment plan input.
