Statement of Purpose The Licensed Vocational Nurse (LVN) assesses clients and provides nursing care in accordance with physicians’ orders and treatment objectives and as instructed by supervisor.

Major Tasks, Duties and Responsibilities Report and record any changes observed in client condition or behavior, including unusual incidents. Identify, report and assist with the management of emergencies.

Take and record accurate vital signs and perform urinalysis testing. Chart routine information e.g. vital signs, client progress and daily activities.

Make rounds every 30 minutes for 72 hours on withdrawal management clients, document and record withdrawal symptoms.

Observe and document client self- administration of medications.

Administer anti- overdose medications as needed

Document client treatment progress.

Conduct medication audits and destroy outdated medications per policy

Interact with clients to support their participation in the program. May provide educational sessions or other groups for clients.

Assist with orientation, admission, transfer, and discharge of clients.

Provide clerical support as directed by nursing staff. Keep charts in order with all forms properly identified.

Follow directions of physicians and registered nurses.

May be required to successfully complete a medication test.

Observe universal precautions, including correct handling of blood borne pathogens, storage of lab specimens, and disposal of infectious wastes.

Remain calm while performing job duties.

Assess and report client behavioral changes.

Identify a crisis. Demonstrate appropriate techniques for interventions, including AMA, behavioral issues, suicide threat, code green, and contraband on the unit.

Make presentations and facilitate educational groups & sessions as needed.

Work as a member of the treatment team, especially in carrying out the nursing care plan and treatment plan.

Maintain professional boundaries and standards.

Treat clients with respect, care and concern.

Recognize personal biases working within diverse populations. Maintain cultural sensitivity at all times.

Comply with confidentiality requirements at all times.

Represent BHS well in all contacts.

Maintain familiarity with BHS policies and procedures and other applicable regulations; performs job duties accordingly.

Carry out appropriate interventions, including responding to emergencies and performing CPR and/or first aid.

May order inventory, and/or stock supplies in the unit.

May be required to assist clients with self-administration of PRN and other medications as ordered by physician. ourteously answer phones, appropriately directing calls and taking messages. Make phone calls for clients as approved by the nurse in charge.

Handle clients’ personal belongings as directed.

Run errands within the facility and respond to requests for assistance from the nurse in charge, the DON, and other authorized staff.

Attend meetings and participates in committees as assigned.

LVN Competencies and Performance Expectations All LVN’s are expected to:

Statement of Purpose The Licensed Psychiatric Technician (LPT) assesses patients and provides nursing care in accordance with physicians’ orders and treatment objectives and as instructed by supervisor. The LPT works under the supervision of an RN in a substance use detox subacute hospital.

Major Tasks, Duties and Responsibilities Make presentations and facilitate educational groups & sessions.

Assist in the assessment, planning, implementation, and evaluation of the nursing care plan.

Assess and document patient treatment progress, medical or behavioral changes, and unusual incidents.

Monitor patient management and treatment. May be assigned to provide continual observation of confused patients.
